APPENDIX A – COMMUNICATION LETTER

We would like to ask permission to allow us to conduct a survey among the Senior High
School teachers and this will include all the subject teachers from the said department.

This is in view of our research entitled, “Bikoy 411: An Online Student Information
Database System”.

The survey will last about 10–15 minutes and would be arranged at a time convenient to
the respondents’ schedule. Participation in the survey is entirely voluntary. All

This questionnaire will serve as an instrument in gathering information on the

effectiveness and benefits of using Bikoy 411 to monitor and access the basic information

of the Senior High School students of Batangas Eastern Colleges.

the information I needed.

boxes are easy to navigate, and also its contents cover the
vital point about the student.
5. Bikoy 411 maintains the students' data privacy, where the
information is kept private and accessible only by the
accounts registered on the website.

APPENDIX C – DOCUMENTATION
Group 3 virtual pre-defense presentation happened last February 20, 2021, that

started at 5:10 in the afternoon and ended at 5:45 pm. The panelists were Dr. Lorelie A.

Galit and Mrs. Minerva S. Magisa. The group 3 researchers were Coronel, Anna Jasmine

R., Garcia, Mary Rose M., Ilao Regie L., Laraya, John Louie B., and Paborito, French

Joana M. Their Research was entitled " Bikoy 411: An Online Student Information

Database System'' that helped the school personnel and teachers to manage the students'

record and information. Chapters 1 to 3 were discussed by the researchers. After the

discussion and presentation of the said chapters, they began to present their output to the

panelist. After the discussion and presentation of the output, the panelists started to give

their suggestions and recommendations regarding the paper. Mrs. Magisa started with the

introduction, she commented that the introduction should discuss the purpose of the

proposed project and not the product itself. She also said that the second paragraph

should be revised its paragraph form because it's too long. Furthermore, she included that

the conceptual framework should have a discussion and the table should be isolated in

one page. In chapter 2, Mrs. Magisa suggested adding more related literature and studies

because the current RRL is not enough to support the study. In conclusion, she

recommended that the respondents should only be the teachers and don't include the

students and parents due to the data privacy act.

On the other hand, Dr. Galit suggested that sop number 1 should be erased in the

statement of the problem because it is similar to sop number 2 they both ask for the

perception of the respondents about the Student Information System. She also

commented that the introduction should not endorse the product and should only be
discussed for the proposed project. In chapter 3, Dr. Galit commented that the research

instrument should describe the instrument used in the study. She also added that the

mathematical measures used in the data analysis procedure were not enough to measure

the level of effectiveness. And both Mrs. Magisa and Dr. Galit suggested changing the

respondents and make only the teachers be the respondents in the study. They also

commented that the format spacing of the bibliography is incorrect. At the end of the pre-

defense, the panelists as well as Sir Ambal congratulated the researchers.
