Case Study 1

Suppose you are the director of a community health organization that serves patients with High
Blood pressure (HBP) in Chesapeake, Virginia. The current prevalence of HBP in your
community is 34.6% (GHR, 2021), and your city has not met the objective of Healthy People
2020 (26.9%). In the effort to meet the aim of Healthy People set for 2030 (27.7%), your board
decided to conduct a needs assessment to determine where the needs are in your city for
proper allocation of your resources.

Questions:
1. What are the kinds of sources of data that need to be collected to
make rational/comprehensive planning decisions?

Based on the review of the data collected, you have located four low-income communities that
have the highest rates of HBP in your city. You are interested in identifying the unmet needs of
the four communities and determining risk factors that contribute to the HBP rate.
