Step 4: Continue with Child’s Information

Fill in your child’s

Name as in Passport.

Year of Birth will be

auto-displayed upon
selecting a valid Day
and Month of Birth.

Please upload your

child’s Passport Bio-
data Page and Birth
Certificate in PDF or
JPG format.

Step 5: Add Sibling Section (Second Child)

Click on button to proceed. If you do not have a second child to

indicate interest, please select “No” at the previous question.

Step 6: Add Sibling Section (Third Child)

If you select “Yes”, the note in green box will be displayed. Please
continue to complete and submit the form. Ensure that you have
provided a valid email address in the form.

Step 7: Parents’ Information

Fill in Father’s Name

based on your child’s
Birth Certificate.

If “Singapore Citizen
(SC)” is selected, the
form will prompt you
to upload the Parent’s
NRIC (Front), NRIC
(Back) and Marriage
Certificate in PDF or
JPG format.

Fill in Mother’s Name

based on your child’s
Birth Certificate.

If “Singapore Citizen
(SC)” is selected, the
form will prompt you
to upload the Parent’s
NRIC (Front), NRIC
(Back) and Marriage
Certificate in PDF or
JPG format.

Step 8: Singapore Contact Details

on whether their
child can be
offered a P1
place.

If you select “Yes”, please proceed to select the

National School that the sibling(s) is currently
studying in.

Read Declaration and click on the

check box to proceed.

Click <Submit> to complete your

online indication of interest.
