Student Deferment of Taking the Summative Assessment

1. I declare that I am deferring taking the Summative Assessment scheduled for October 2020 to a
later date.
2. I understand that my Course Instructor in AY2020-21 First Semester is no longer under any
obligation to guide me in the Formative Assessments.
3. I understand that the Google Classroom or Daigler Class will remain available to me until the
first semester of AY2021-22 to help me prepare for the Summative Assessment on my own.
4. I understand that I have a maximum of three chances to obtain a passing rating in the
Summative Assessment for this course.
5. I understand that once I pass the Summative Assessment, I am no longer eligible to re-take the
Summative Assessment.
6. I understand that my chances of taking the Summative Assessment are during the published
schedule provided by the Chemistry DSAC. The dates are in a) Nov-Dec 2020, b) Jan-Feb 2021,
and c) Aug-Sep 2021.
7. I understand that I have to pay a “bother fee” for any subsequent re-takes of the Summative
Assessment. I shall contact the Finance Office thru any of the following: 1)

8. I shall contact the Chemistry DSAC (dsac.chem@addu.edu.ph) at least two (2) weeks prior to the
announced schedules to arrange the Summative Assessment or retakes, if necessary.
9. I shall use only my official AdDU email address to contact the Chemistry DSAC.
