GUIDELINES FOR LESSON EXEMPLAR MAKING

1. The lesson exemplar output will be the Midterm examination for SCI1 (Output-based).
2. The topic for the lesson planning will be given by your instructor. The document file of the said topics (3 topics to choose
from) will be uploaded in the Open LMS. Choose only one of the said topics.
3. Download the template of the lesson exemplar. It will serve as the official format of the said output.
4. Follow the 5Es model of instruction for the said lesson exemplar making.
5. Provide 2 or 3 lesson objectives.
6. Submission of the said output is strictly in document form. Photos and screenshots are not accepted.
7. Use Arial, Arial Narrow or Times New Roman as the font style. The font size must be 12.
8. The line spacing must be one point five (1.5).
9. Submit the said lesson exemplar output in the scheduled day of your examination (specifically in the time of the exam).
10. The instructor will only check and record the score of the lesson exemplar if the students were able to submit their
examination permit.
11. Use the rubrics as your guide for developing the lesson exemplar. It will be posted in the Open LMS 3 days before the
submission.
