On the examination day, examinees must submit a Medical Certificate at the designated area for the medical team. It must be issued by a licensed government physician from the Barangay/Municipal/City/Provincial Health Office, or other government health monitoring center/office where the examinees reside. It must clearly state that he/she is COVID-19 free. Said Medical Certificate must be issued within 72 hours before the examination day (Medical Certificate issued by private health institution shall not be accepted). ‘An examinee who fails in an examination, or after being admitted to take said examination, fails to take it for one reason or another, need not resubmit the requirements in the event he/she applies to take the same examination in the same examination center. In such cases, the applicant will only have to: (a) submit the ADMISSION SLIP of the last examination applied for; (b) fill out a new application form and index card with ID pictures; and, | (©) pay the examination fee.
