JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ESTRELLA BAUTISTA LOPEZ and CHARLOTTE GALVEZ both legal age,
respectively, Filipino citizens, and residents of 48 B MONACO ST., CIUDAD GRANDE
ROSARIO, PASIG CITY and 61 MONACO ST., CIUDAD GRANDE ROSARIO, PASIG
CITY, Respectively, after having been duly sworn to in accordance with law, hereby
depose and state THAT:
1. We have known personally MERCY CACERES OLVIDO (maiden name) born
on MAY 9, 1960 at NEGROS OCCIDENTAL;

2. In the record of my pertinent documents including my IDs my date of birth

appeared as “May 9, 1962” but in the record of my certificate of live birth
my date of birth appeared as “May 9,1960’’ which is the true and correct;

3. The date of birth ‘’May 9,1962’’ and ‘’May 9, 1960” appearing in the
records refer to one and the same;
The discrepancy and disparity in the inadvertence of the true date of birth
was purely due to excusable human negligence or error;

4. We executed this Affidavit to attest to the truth and veracity of the above-
mentioned facts and for whatever legal intent and purpose this may serve.

IN WITNESS WHEREOF, we have hereunto affixed our signature this 17th

day of September 2021, at Pasig City Metro Manila.
