August 12, 2021 DIVISION MEMORANDUM jo.__T)f 2021 PREVENTING ERASURES ON DEPED OFFICIAL DOCUMENTS To: _ Division Office Personnel Public Elementary and Secondary School Heads ‘Secondary School Sr. Bookkeepers /Disbursing Officers Elementary School Sr. Bookkeepers Other Concerned Personnel 1. _ It has come to the management's attention that official documents like the Disbursement Vouchers, Purchase Orders and its supporting documents bear some erasures which diminish and question the authenticity and reliability of the information the documents contain, hence this Memorandum. 2. It is therefore advised, and shall now become part of the policies of this Office, that the processing of the Disbursement Vouchers, Procurement Documents and all reports of the Division Office and those that are coming from the field, specially the Liquidation Reports with all the supporting documents, shall be submitted free from erasures or unnecessary marks that might suggest tampering of vital information. 3, _ It is also suggested that proper review must be done prior to printing of the Disbursement Vouchers, Procurement Documents and of various reports to minimize wastage of office supplies. Supporting documents to the Liquidation Reports coming from various schools must be limit to only two (2) copies. 4. For your information, preferential attention, and strict compliance.
