Dear Mrs. Rowena Monroy,

Good day! To God be the Glory!
This is to respectfully refer my client: Jhon Mark Calisay, 6 years old for temporary shelter in
your institution and have an immediate medical consultation to further his treatment.

My client is from Sitio Malobago, Brgy. Matarinao, Salcedo Eastern Samar, Jhon Mark
had an accident that caused his left hip’s dislocation subsequently causing his inability to walk
without support. He needs an immediate medical intervention to restore his normal physical
ability. They were abandoned by his father and his family has no means to pursue his medical
treatment due to a very poor economic condition. From the time that their father left them, Jhon
Mark stayed and was raised with the help of his maternal grandparents at Brgy. Matarinao
Salcedo E. Samar. The family has a harmonious relationship with each other. They live from
hand-to-mouth existence, his grandfather as the breadwinner of the family who is now engaged
in a small-scale fishing. While his grandmother is the one who takes care of Jhon Mark and his
sister. Jhon Mark’s physical disability could’ve been treated if only his family has the means of
consulting to a Doctor immediately after the accident. As much as his family wanted to treat him,
they could only afford to go a “hilot” in their community due to their distant location and poor
economic condition. The child’s future could even be brighter if he could be helped get through
his disability. He needs immediate medical intervention to save him from permanent disability. I
hereby refer the child to temporarily stay in your institution and have an immediate medical
consultation to further his treatment.
