4. The change of the color of the subject vehicle is made in good faith and
not to conceal any offense and not intended for any illegal or unlawful
purpose but solely due to personal reason and to upgrade the physical
appearance of the above described motor vehicle.

5. I am executing this affidavit to attest the truth of the foregoing for all
legal intents and purposes and more specifically to cause the registration
of the above change of color of my motor vehicle on its Certificate of
Registration before the Land Transportation Office (LTO).
