    A snakebite victim was admitted and their vital signs were monitored for 24 hours. They received an infusion of 4 ampules of anti-snake venom over 500mL of fluid and were watched for adverse reactions, as sensitivity testing is unreliable. They were also given antibiotics to prevent infection, a pain reliever, and omeprazole to prevent ulcers. Blood tests were done and fresh plasma administered if coagulation was affected. The patient was told to avoid injections and NSAIDs for a week.

    Snake bite

     Admit the pt, Reassurance of a snakebite victim, Anxiety state increases


    venom absorption, Pass IV line, Monitor the vital signs for 24 hour
    ________________________________
    Anti snake venom infusion, 4 ampule in 500l Clopidogrel , watch the
    pt for adverse reaction. Do not perform sensitivity test as it poorly
    predicts anaphylactic reactions.
    Infusion R/L – 1 L IV BD
    Treat with antibiotic if there s any infection in bite sit or if the wound
    looks contaminated or necrosed.
    Tab danzen forte (Serrapeptase ) 1 tab PO OD f there s swelling
    Tab digesic forte (Dextropropoxyphene +paracetamole) SOS for pain
    and fever
    Infusion omeprazole (PPI) diluted in 100m of normal saline or
    dextrose water OD or Cap omeprazole 20mg OD-——to prevent
    stress ulcer
    Fasciotomy may be considered if the swelling increasing and there s
    risk of Compartment syndrome (call to orthopedic department for
    expert opinion if swelling increasing)

     Investigation: FBC, PT/APPT, CT, BT, Urine R/E, ALT/SGPT
     If PT/APT/INR is deranged then arrange fresh frozen plasma
     Injection Vit-k IV.0D for 3 day
    ____________________________
     No I/M injection, No NSAID for one week
