REQUIREMENT for NOTIFICATION Source

1. Valid LTO of the manufacturer (amended to add PC as SCD
client/trader and the product being notified)
2. Notarized Supplier-PC Agreement SCD

PD/QA/TSD

REQUIREMENT for NOTIFICATION Source

3. Quantitative and qualitative composition of product (indicate PD
restricted material and limits)
4. Documentation to substantiate product claims; CoA of Active PD / TSD
Materials and its mechanism of action (as maybe asked by
evaluator)
5. Clear and complete loose labels or artworks of all packaging PD / QA
sizes, as applicable, in Filipino or English language
6. Prepare PIF PD/QA/TSD
