LEGAL SERVICES CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

I, _________________________, hereby acknowledge that I have engaged the legal

services of Attys. _____________________________ in the Labor Case entitled
“_____________________________________, who in turn have rendered exemplary legal
services on my behalf.

That through the efforts of my legal counsel, the National Labor Relations Commission
(“NLRC”) ruled and found me entitled to monetary claims in the total amount of Two Hundred
Fifty Thousand Pesos (Php250,000.00) plus ten percent (10%) attorney’s fees.

That by virtue of my legal counsel’s endeavors, I will be able to collect the money claims
in the total amount of Two Hundred Fifty Thousand Pesos (Php250,000.00) through a UCPB
Check to be issued by the NLRC.

That upon receipt, deposit in my bank, and bank clearing of said UCPB Check, I promise
to pay my legal counsel the amount of fifty thousand pesos (Php50,000.00) as consideration for
the legal services rendered to me and reimbursement of the filing fees, costs and other expenses
incurred by said counsel for the effective prosecution of my Labor Case.

That the payment of this fifty thousand pesos (Php50,000.00) fee to my legal counsel
shall be made no later than 29 September 2021.

That my legal counsel shall continue to exert efforts to execute the remaining ten percent
(10%) attorney’s fees or the amount of twenty five thousand pesos (Php25,000.00) which I have
been ruled by the NLRC as entitled to.

That upon collection of the said attorney’s fees, the same will be turned over to me and I
promise to pay my legal counsel twenty percent (20%) of the amount collected.

That the legal obligations contained under this LEGAL SERVICE CONTRACT shall
be binding and effective to herein parties’ successors, heirs and assignees.
