Dear Veteran,
Thank you for taking the time to attend the Posttraumatic Stress Disorder (PTSD)
evaluation. There are two goals for this evaluation:
1. To determine if you have PTSD related to one or more traumatic events that
occurred while serving in the military. (If you are already service-connected for
PTSD, the clinician will determine what additional evaluation is necessary.)
2. To discuss treatment options for PTSD or to receive other appropriate referrals.

IMPORTANT: This evaluation is NOT for service-connection purposes. If you are not
interested in PTSD treatment, please let us know.
In order to meet the above goals, you will first complete the intake paperwork and then
meet individually with a clinician who is trained in evaluating PTSD. Please be prepared to
discuss what you are hoping to achieve today and your interest in treatment. Again, if you
are not interested in treatment, please let us know right away.
On rare occasions, we have a very high number of veterans seeking evaluations and not
enough clinicians. If this occurs, the wait may be longer than anticipated or we may ask you
to reschedule your evaluation at a time that is convenient to you. We will make every effort
to avoid doing this, however our top priority is to conduct the evaluation and determine
your treatment needs with as much attention and care as possible.
If you have any questions or concerns about the above process, please ask one of the
facilitators.
