WITHNESSETH – That:

WHEREAS, the parties herein are the children and legal heirs of deceased
SALVADOR A. AVISO, born on July 7, 1932 and who died intestate on November
21, 1990 married to ESTILITA B. AVISO, born on May 5, 1934 and who died
intestate on September 12, 1915.

WHEREAS said deceased, at the time of his death, left a certain real properties
and covered with TCT NO. 380105;

That the said TCT NO. 380105 was loss due to unavoidable circumstances
when it was unfortunately misplaced, We search for it but it was never found. All
diligent efforts exerted to locate and retrieve it were in vain, hence for all legal intents
and purposes it is now deemed lost and beyond recovery;

That We are executing this affidavit to attest to the truth of the foregoing facts
and for the information and guidance of the proper authorities concern and to facilitate
our affidavit of loss title to mentioned above and for whatever legal purposes and
intents it may do serve.

FURTHER AFFIANT SAYETH NAUGHT
