PRESCRIBING NEW DOCUMENTARY REQUIREMENTS FOR THE
PHARMACISTS LICENSURE EXAMINATIONS,
WHEREAS, Republic Act (RA) No. 10918, also known as The Philippine Pharmacy Act, states
that applicants for registration as Pharmacists shall take and pass the licensure examination
unless otherwise provided by law;
WHEREAS, Section 14, Article Ill of RA No. 10918 provides for the qualifications of applicants
for the Pharmacists Licensure Examinations, to wit:
1. A citizen of the Philippines or of a foreign country which has a law or policy on
reciprocity for the practice of the pharmacy profession;
2. Of good moral character and reputation;
3. Adegree holder of Bachelor of Science in Pharmacy or its equivalent degree conferred
by a Higher Education Institution in the Philippines or an institution of learning in a
foreign country duly recognized by the Commission on Higher Education; and
4, Has completed an intemship program approved by the Board, pursuant to such
guidelines as may hereinafter be promulgated, in consultation with the duly recognized
associations of pharmacy schools and the CHED.
WHEREAS, the Professional Regulation Commission (Commission) issued Memorandum Order
No. 04 (s 2016) on the “Streamlining of Procedures in the Preparation of Board and Commission
Resolutions and other Official Issuances involving Policy Decisions and Directives; Actions and/or t
Decisions on Applications for the Licensure Examination, Registration without Examination,
Issuance of Certificates of Accreditation, Certificates of Compliance (COCs), Certificates of
Authority to Operate, Certificates of Registration of Corporations and Partnerships for the Practice
of Profession and other Similar Applications”,
WHEREAS, the Board issued Resolution No. 119 (s 2017) on the “Designation of Board Focal
Person for the Evaluation and Approval of Applications for the Licensure Examination,” with
Annex A on the list of documentary requirements for the Pharmacists Licensure Examination;
WHEREAS, the Board deemed it necessary to review, rationalize and update the existing
requirements for the licensure examination; \
NOW THEREFORE, the Board has RESOLVED to prescribe the new documentary requirements y
for the Pharmacists Licensure Examination, to wit:
1, Original and photocopy of the Philippine Statistics Authority (PSA)- issued Certificate
of Live Birth;
2. Original and photocopy of the PSA- issued Marriage Certificate (For married female);
3. Certification of Good Moral Character issued by any of the following: barangay, church,
school of pharmacy or employer;
4, National Bureau of Investigation (NB!) Clearance;
5. Certified true copy of the Transcript of Records (TOR) in B. S. Pharmacy or its
equivalent degree conferred by a Higher Education Institution (HE!) in the Philippines
or an institution of learning in a foreign country duly recognized by the Commission on
Higher Education (CHED);
6. Certificate of Completion of the internship program duly signed by the dean/program
head;
7. Other documents as may be reasonably required by the Board.
In case of three (8) failures, the applicant has to submit a Certificate of Completion of
Refresher Program duly signed by the dean/program head of the school.

RESOLVED FURTHER, that herein new requirements shall apply starting the March 2019
schedule of the Pharmacists Licensure Examination,
The list of documentary requirements specified in Annex A of Board Resolution No. 119 (s 2017)
is hereby repealed
This Resolution shall take effect after fifteen (15) days following its publication in the Official
Gazette or in any newspaper of general circulation
