Dear Kahok Family:

The Collinsville School District is currently evaluating options for remote learners
for the 2021-2022 school year.  Consistent with the guidance from the CDC, IDPH
and ISBE, families of students will need to secure a licensed physician’s statement
to qualify for exclusive remote instruction and will be required to complete a
district approval process. Students who are at increased risk of severe illness
(including those with special health care needs) or who live with people at
increased risk may be given the option of remote instruction provided an approved
statement from a licensed physician’s statement accompanies the request. In
addition, if the Governor lifts the disaster declaration that is currently in place,
elective remote learning would no longer be an option for students in the
Collinsville Unit 10 School District.

To assist our District in planning for in-person and remote learning, we ask
families who will likely pursue an exclusive remote learning plan for their
child(ren) next year to complete this Remote Learning Survey on or before April
30, 2021. Please complete this survey ONLY if you believe that your child will
qualify for remote learning next year. Please complete a form for each child in your
household who you believe will qualify for an exclusive remote learning plan for
the 2021-2022 school year.

The District will evaluate requests for remote instruction according to Section 504
evaluation procedures, state and federal law and Illinois State Board of Education
(ISBE) rules.

Please understand that completing the survey will not qualify you for a remote
learning plan for your child(ren), but will benefit the District in planning for the
2021-2022 school year.
