Please fill out and return this form no later than Wednesday October 13th, 2021 so that lesson
groups can be formed and scheduled by the first lesson on OCTOBER 27th.

If there is a year-long commitment that prevents your musician from attending Wednesday evening
lessons, there is the potential for a mixed session after school. This lesson group will have a
maximum capacity limit and if it is full and Wednesdays do not work for your family, it will be your
responsibility to coordinate private lessons if your students wishes to continue participating in band.

Potential alternative after-school lesson days are Mondays and/or Thursdays:

If your musician is unable to make any of the offered lesson times at NBCS, they will need to take a
lesson elsewhere to participate in the Band program. (Potential Lesson Instructors available through
Music and Arts in Manchester, the Manchester Music Mill, or the Manchester Community Music School.)
