Name of Activity: “Gulay para sa mahabang Buhay”
Target Participants: Grade 12 students (30 in class)
Objective: The students will be able to make a creative vegetable plant box using recyclable
materials that will benefit the community and promote eating healthy habits among the
residents.
Instruction:
1. The class will be divided into 6 groups with 5 members each.
2. The teacher provides a printed rubric to be given together with the printed instruction of the
activity.
3. Specific Instructions:
a. The students will make use of recyclable materials in making their vegetable plant box like
trays, water container, plastic bottle, woods, and etc. They can choose three (3) vegetables to
plant as long as it is easy to grow.
b. They must make use of their creativity and innovation skills in making their vegetable plant
box. They can add design to their plant box to make it look attractive and put a message about
eating healthy foods.
c. They must put a label on the types of vegetable they plant and its health benefits to us.
d. For the rating of output, the process of collecting the materials, making of the output and the
finish product must be documented (i.e. pictures, journals, reflections, etc.). These learning
evidence will be placed in a portfolio with the names and photos of the members in it and their
contribution to the group.
e. The finish product will be donated to the barangay through brgy captain or brgy
representative.

Name of Activity: “Awareness for Everybody Happiness”
Target Participants: Grade 12 students (30 in class)
Objective: The students will be able to make a creative mural art in the community promoting
about safety protocols such as proper wearing of face shield and mask, observing social
distancing, proper hands washing, and ways to prevent the covid virus.
Instruction:
1. The teacher will assign 3 student leaders to facilitate the whole class in deciding an idea or
design for their mural.
2. The teacher provide printed instruction of the activity.
3. Specific Instructions:
a. The student leaders with the assistance of the teacher will coordinate with the brgy captain
or brgy representative on where the area they will paint their mural art.
b. The student must make use of their creative and imaginative skills to create a beautiful mural
art for the community. They can use used paint or paint brush in making their project.
c. The mural art must convey a message about the different safety protocols in order to spread
awareness in the community.
d. For the rating of output, the process of making the output and the finish product must be
documented (i.e. pictures, journals, reflections, etc.). These learning evidence will be placed in a
portfolio with the names and photos of the members in it and their contribution to the group.
e. The finish product will be shown to the community.
