PERSONAL FITNESS PROGRAM


PLAN CONTRACT
I, Karen P. Valle, am made this contract as
evidence that I will follow everything that is
written on my program plan to be a healthy
individual. If ever I failed to perform my
intended tasks, I will stop using my phone until
the time my parents wound finally allow me
own it again.
Starting on the month of February 2021 to
August 2021.

PERSONAL FITNESS PROGRAM PLAN
I am challenging myself to follow everthing ritten on my
program plan in order to achieve these following goals.
LONG TERM GOAL
 Slim, fit and healthy body for the next six months.
SHORT TERM GOAL
 Jog for at least 30 minutes
 Drink a lot of water a day
 Avoid eating unhealthy foods
 Sleep 7-8 hour every night
 Low weigh
 Regular exercise
This program plan of mine will bwgan on February 2021 and it
includes the following schedules for my long term and short
term goals. Every goal will achieve a reward once I
successfully completed every task on my target goals. These
are my list of rewards.
