Description of the Prototype

1. Patient Reservation Module

Before the patient have a reservation, the reservation form need to fill-out. The basic details of the
patient namely, fullname, age, gender, address, email, contact-no, username and password. From
there, patient successfully create a membership account.

The sign-up and created a membership account, the patient can now login using the username and
password.

2. Patient Dashboard Module

After the patient login into the website, the page will be the dashboard module. The dashboard will
display the patient page which contains the date and services of appointment . the patient will select
the date and services offer to make reservation . the account on the top contains the schedule where
patient can view the schedule created. Also the log-out. The patient can also update the info if there
are some changes. From both side of the dashboard display the note , list of services offer, the date
and office hours together the info of midwife.

Before the patient submitted the appointment, there will be a confirmation if the patient need to
change the selected date or services then submit. After submitting the appointment, the message will
display that the appointment has been set on the date selected.

3. Admin Dashboard Module

After the multiple or BHW login into the system, the first page will be the admin dashboard module.
The sidebar dashboard will display time , date, home, schedule for today, schedule , services , user
account, note and patient records. The icon setting contains the log-out button.

The midwife or BHW can view the schedule for today that contains the patient appointment. From
the table, the name will be displayed. The midwife or BHW will approve the pending appointment of
the patient

All the appointment of the patient will be store in schedule. It will display the table containing, the
name of member , date, services , status and action. The midwife or BHW will approve the pending
appointment of the actions the midwife, or photo will approve the pending appointment of the
patient. There are also a search bar which they can easily find the same name of the patient.

3.Admin Login Module

The midwife or Barangay Health Worker (BHW) needs to login with their own account. The username
and password are needed to login and use the functionality of the system.

From schedule table, the Midwife or BHW an add services and display the services offer. They can also
delete or update the services.
