JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, JOSEFINA APAS PONCE and ROGELIO PADAYOGDOG, both of legal age,
Filipino citizen, married and residents of Panoypoy, Consolacion, Cebu; having been duly
sworn in accordance with law, hereby depose and state:

1. That we have personally known the person of BETALIANA ABELLA REYNES,

because she and the members of her family are our long time friend and neighbor;

2. That we have personally known that BETALIANA ABELLA REYNES died on August
8, 2021 at Panoypoy, Consolacion, Cebu, Philippines;

3. That recently, it was discovered that there are discrepancies in some of the personal
files and official records of BETALIANA ABELLA REYNES;

4. That we hereby affirm and confirm that the following names refers only to one and
the same person, to wit:

a. BETALIANA REYNES per her Certificate of Death;

b. BETALIANA PADAYOGDOG per Name of Mother in the Baptismal Certificate

of her son, RAYPEE PADAYOGDOG;

c. BETALIANA AVILA PADAYOGDOG per Name of Mother in the Certificate of

Live Birth of her son, RAYPEE PADAYOGDOG;

d. BETALIANA ABELLA REYNES per her Certificate of Indigence;

5. That the same discrepancy was purely a clerical error, with all good faith and without
the intention to defraud any person, including the government of the Republic of the
Philippines;

6. As such, we execute this Affidavit to certify or attest to the truth of the foregoing facts
and for whatever legal purpose that this Affidavit may serve.

IN WITNESS WHEREOF, we have set our hands this 19th day of October 2021 at
Mandaue City, Philippines.
