1. SUBJECTIVE: SHOOTING INCIDENT

2. DTPO: OOA 7:00AM of March 14,2021, N0. 123, Bangon St. Brgy. Nakahiga, Lumaway City,
Maguindano

3. VICTIM/SUSPECT/FIREARMS INVOLVED:
Victim: Juan M Dela Cruz, 46 years old, married, laborer, and a resident of Bangon St. Brgy. Nakahiga,
Lumaway City, Maguindanao
Suspect: Pepito Y Manaloto, 47 years old, married, farmer also a resident of Bangon St. Brgy. Nakahiga,
Lumaway City, Maguindanao
Firearms used: .45ACP cal.9mm Gen 5 Glock 19 with SR.6969AWS registered under the name of Pepito Y
Manaloto 47 yrs of age (DOB. February 29, 1974), resident of Bangon St. Brgy. Nakahiga Lumaway City,
Maguindanao. A holder of a Gun License with no.19-87000 valid until December 25, 2022.

4. NARRATIVE:
On March 14, 2021 at about 7:00 o’clock in the morning a shooting incident transpired at NO. 123,
Bangon St. Brgy. Nakahiga, Lumaway City, Maguindano wherein victim one Juan M Dela Cruz, 46 years
old, married, laborer, and a resident of Bangon St. Brgy. Nakahiga, Lumaway City, Maguindanao, was
shot by suspect Pepito Y Manaloto, 47 years old, married, farmer, and a resident of the same place. As a
result thereof, the victim sustained multiple gunshot wounds in the different parts of the body and was
rushed to Maguindanao Provincial Hospital for medical treatment but was pronounced dead on arrival
(DOA) by the attending physician thereat. The suspect was arrested at his house during hot-pursuit
operation conducted, after further inspection later on the firearm used was identified to be a .45ACP
cal.9mm Gen 5 Glock 19 with SR.6969AWS. The suspect is now under the custody of this station for
investigation and proper disposition. The cadaver of the victim was moved to Regional Crime Laboratory
Office, P.Acharon Ext, General Santos City, South Cotabato for autopsy examination.

5. STATUS OF THE CASE: Under Investigation

6. ACTION TAKEN: Diligent Efforts were being exerted to locate and inform the relatives of the deceased
victim. Relayed to PIDMB.
