Dear Sir:

This is concerning with the Effectivity Date in Agency Remittance Advice Form A. We would like to clarify
as to what date we should use between the Date of Appointment and Date of Assumption of duty of the
employee. This is in instances where the Appointment date is different from the date of effectivity
stated in the Certification of Assumption of Duty of the employee.

In our employee’s case, she started reporting on the date of assumption of duty. What date should we
follow?
