I Aim
The aim of this policy is to provide new hires a proper guideline on physical medical
examination prior to the commencement of work with (the “Company”) as part of the On-
boarding process.

II Scope
This policy applies to all new hires commencing work with the Company.

III Objectives
1. To ensure all new hire(s) are physically fit for work;
2. To fulfill any insurance requirements for insurance coverage and/or work visa
application, if applicable.

IV Definitions

1. Medical Checkup will consist mainly of x-ray, drug, blood, urine and stool tests as
applicable per country and regulatory requirements.

V Roles & Responsibilities

1. Human Resources Department will be responsible for the issuance of medical chit to
the potential new hire(s) for facilitating the physical medical examination with the
Company’s appointed medical clinic.

2. Upon the receipt of the medical chit, new hire(s) is/are to attend the physical medical
examination and cooperate in providing all body fluid/sample, at the Company’s
appointed medical clinic five (5) business days prior to the commencement of work.

VII Consequences

The Company and/or regulatory of relevant countries, take serious view of potential hire(s)’
well-being prior to commencement of work and/or issuance of work visa. Failure to adhere,
cooperate or comply may result in non-commencement of work or non issuance of work visa.
