appearing in her death certificate;

2. She is also known to us as Ma. Cristina Alarcon Mabesa, as appearing in her

record with Climbs Life and General Insurance Cooperative;

3. Lately, it was found out that the first name of said person is MARIA
CRISTINA, in accordance with her birth certificate, and not MA.
CRISTINA;

4. We hereby declare and attest that the names Maria Cristina Alarcon Mabesa
and Ma. Cristina Alarcon Mabesa refer to one and the same person;
5. We are executing this Affidavit to attest to the truth of the foregoing facts
and to use the same for whatever legal purpose it may serve.

4. We hereby declare and attest that the person of whose birth year is 1982 as
appearing in his birth record and the Arnold Amoroso Mabesa whose birth
year in the records of the above-stated insurance cooperative as 1983, refer
to one and the same person;

5. We are executing this Affidavit to attest to the truth of the foregoing facts
