ENROLLMENT PROCEDURE

STAGE 1 EARLY REGISTRATION

A. Remote Early Registration
 Message the Focal Person for enrollment for the Google Enrollment Form.
 Access the online enrollment form provided by the school.
 Fill-in the relevant data required in the form.
 Submit the accomplished form.

B. In-Person Early Registration

 Accomplish the Modified Basic Education Enrollment Form (MBEEF) with the relevant data required.
 Submit the enrollment form and the complete requirements per grade level.

C. Early Registration thru Kiosks/Drop Boxes in Catchment Barangays

 Observe public health and safety protocols upon entering the barangay catchment premises (i.e., sanitizing,
temperature checks, health declaration form and physical distancing).
 Get a copy of the Modified Basic Education Enrollment Form (MBEEF) from the kiosk in the catchment
barangays.
 Accomplish the form by filling in all the fields with the learner’s accurate data. Just write N/A in case the
question is not applicable.
 Once finished, submit the form together with a copy of the learner’s Certificate of Live Birth in the
corresponding drop-box for accomplished MBEEF.
 Leave the premises immediately.

D. For Transferees and Balik Aral Enrollees

 Contact the receiving/preferred school directly through its published enrollment contact details.
 Access the Modified Basic Education Enrollment Form (MBEEF) either through online or in-
person.
 Accomplish the enrollment form with the relevant data required.
 Submit the enrollment form and the complete requirements.

STAGE 2 LIS ENCODING

 Consolidate data collected from kiosks/drop boxes in catchment barangays for encoding.
 Encode the data for incoming Grade 7, Grade 11, Transferees and Balik Aral Enrollees in the system
including those consolidated data collected from kiosks/drop boxes in catchment barangays.
