CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF SERVICE, is made, executed and entered into by:
The Local Government Unit (LGU), with office address at Barangay 02, Poblacion, San Luis, Aurora
represented by its Mayor ARIEL A. DE JESUS hereinafter referred to as “First Party”.
-AND-
Mr./Ms. _________________________________ Filipino of legal age, married/single/widowed and resident of
_____________________________________referred to as the “Second Party”.
-WITNESSETH-
WHEREAS, pursuant to the provisions of the Local Government Code of 1991, the LGU is mandated to promote
the general welfare of its constituents and ensure the efficient and effective provision of the basic services and facilities;
WHEREAS, to fulfill its mandate the LGU provides and implements various program, project, services and
activities funded under the General Fund Annual Budget of the province;
WHERAS, these development programs, projects and efforts of the LGU are being carried out by its concerned
departments;
WHERAS, due to the exigency of public service and manpower requirement of each office/department
undertaking such programs, project, activities and services, hiring of qualified personnel under Contract of Service in
imperative;
NOW THEREFORE, for and in consideration of the foregoing premises, the parties hereto agreed as follows:
Section I. SCOPE OF WORK
The Second Party shall be designated as _________________________ and as such shall perform the following
tasks and services hereunder enumerated:

 Perform _______________________________works and other duties as may be assigned by the immediate

supervisor.
Section 2. PLACE OF ASSIGNMENT
The Second Party shall be assigned in the _______________________________ San Luis, Aurora and to places
within AURORA Provinces where his/her service is required.
Section 3. RENUMERATION
For services to be performed, the First Party shall pay the Second Party a rate of Php. 320.00/day. In
addition, the Second Party is entitled to reimbursement of actual expenses incurred (i.e. transportation, accommodation,
etc.) should he/ she be directed to attend trainings to areas outside of San Luis, Aurora. All payment under this Contract
shall be subject to applicable accounting and auditing rules and regulations.
Section 4. OPERATIONAL/TECHNICAL DIRECTIONS
Performance of all work embodied herein stipulated shall be subject to the operational/technical direction of the
________________________________.
Section 5. EFFECTIVITY
This Contract shall be effective for the period beginning ________________________________ to
________________________ in case of force majeure or similar event or condition which may delay or prevent the
timely completion of the service embraced in this contract, the Second Party shall promptly report the same to the

Concerned LGU department specifying in writing the length of the time involved, the cause or causes of delay, and in
subsequent implication on the entire timetable, work schedule and budget for the program.
Section 6. TERMINATION
The First Party has the right to terminate the service of herein hired personnel upon completion
termination or dissolution of the project and/or program, or for unsatisfactory performance.
