Dear Gentlepersons:

On behalf of my client, Dr. Simon Allen S. Aranas (“Dr. Aranas”), I am writing to

you to report the falsification of the attached Medical Certificate (attached as Annex
“A”). Among others, such Medical Certificate bears the following: 1) illegal use of Dr.
Aranas’ name; 2) unauthorized use of Dr. Aranas’ Medical License (“PRC”) and
Professional Tax Receipt (“PTR”) Number; 3) falsification of his signature; and 4) false
contents.

To briefly narrate what has transpired regarding the above, the following are
essential:

1. Last 2 August 2021, Dr. Aranas received the attached falsified Medical
Certificate from the Secretary of one of his clinics Dr. Goodhealth Family
Clinic (“Clinic”) located at 444 JP Rizal St cor P. Burgos St., Sto. Niño,
Marikina City.

2. The Secretary obtained a copy of the same through a caller (“Caller”)

inquiring about their Home Service Fees for Covid-19 Swab Testing who
alleged that a relative was swabbed for a much lower price. The Caller
alleged that a certain Ronald Añonuevo (“Mr. Añonuevo”) had
administered the swab test and that Dr. Simon Allen S. Aranas was the
signatory to the Medical Certificate bearing the test results and the alleged
examination of a certain Arian V. Datog.

3. It bears stressing that the Medical Certificate was in fact falsified due to
the following:

To reiterate, Dr. Aranas did not examine the person named in the Medical
Certificate and did not assess any swab test results in relation to the said person. A
certain Ronald Añonuevo has been using his details and administering swab tests
without authority.

Again, this report is being submitted to the Department of Health to bring to its
attention the illegal scheme described above which preys on innocent doctors like my
client. We hope that the proper punishment be meted out unto the perpetrators.
