WAIVER FOR PHYSICAL CLASSES

To Whom It May Concern:

I, ________________________, am writing this waiver signed with the consent of my

guardian, ________________________, who allowed me to have a physical class.

This waiver stands as an agreement that Philippine Engineering and Agro- Industrial
College, Inc. will not be held responsible concerning my health, safety and security if worst-case
scenario arises. But as per imposed by the school’s protocol, the faculty assigned to the subject
will still maintain a number of students not exceeding 20 students in each room. During class
hours, strictly no pictures taken to be posted in any social media platform. In addition, both the
instructor and the students will practice minimum health protocols such as social distancing and
wearing of face masks during classes to assure the maintenance of my safety, my co-classmates
and instructor.

Any of my future unnecessary actions outside the premises of the school will be out of
school’s concern and liability. Therefore, whatever wrong doing I will be doing is done by me
only and the school will not be blamed.
I, together with my guardian, fully understand and agree to the conditions provided.
