1. History of Sub Assy:

Date & HMR of fitment: No previous record/incident of LHS axle failure or fitment

2. Present problem: Cat16M/2 motor grader met with an accident and its front LHS
axle got broken and detached from the machine during operation at 30 mtr dump due
to wrong operation & application.

3. Details of observation & inspection done at site:

The under foot condition was found very much bumpy & boulders. The operator has
apparently operated the grader at a speed not favorable for that rough ground
condition and has caused a heavy impact on the front axle spindle and got sheared
under load and fell apart. When we inspected the broken spindle we found 60% of its
cross sectional area had earlier crack before it got sheared today. This was an original
axle and not a repaired one.

4. Action taken:
We recommissioned this grader on the same day by fitting another old axle removed
from breakdown grader Cat 16M/1 (BD for transmission).
Now the broken axle is being repaired in our lathe workshop for fitting back in Cat
16M/1 and also we requested the purchase department to arrange new front axle parts
against our PO made earlier.
