Dear Sir / Madam,
I have applied for a fresh passport at the PSK Ghaziabad. The application reference number is xxx. My name as
mentioned on the Class X certificate and other documents do not match. The class X certificate mentions my name
as “Dinesh Mohan” while all other documents mention it as “Dinesh Mohan Gupta”. I was aware of this so I
attached the paper clipping of two leading daily newspapers and the affidavit as per Annexure E with my application
along with other required documents. At the PSK, I was informed that in order for my application to be processed, I
would need to get the correction of name published through the Gazette of Govt. of India, since I am a central
government employee. However I also came to know that after publication of a recent circular (which I could not
get hold of) the Gazette publication is required only for employees who are of Gazette officer rank and above. So as
I am a class IV employee I understand it is not required in my case. Please clarify the correct requirements.
