To whom it may concern:

To the best of my knowledge, Drew S Wisniewski does not carry a diagnosis of

Raynaud's disease. This is not found in his Swedish chart, he denies a prior diagnosis
of this and denies any symptoms suggesting this diagnosis at this time.

I am his current PCP, however I am taking over for his past provider and have not yet
seen this patient in person. I have however, discussed this issue with him over the
phone on 03/07/19.

Please remove this diagnosis from his file.
