Affidavit of Discrepancy

I, Eupricinia Almachar, of legal age, married and resident of Villamar,

Caoyan Ilocos Sur, do hereby depose and say that;
1. That the name indicated in my birth certificate issued by the Civil
Registry of Vigan City is Euprisima Almachar .The Certification issued
by the Civil Registrar of Vigan City is hereunto attached as Annex “A”.
2. But ever since childhood, my friends and acquaintances have been
calling me Concepcion and I have been using the name Concepcion
Rivad in all my personal transactions, records and communications;
3. That the Purisima Ribad and Concepcion Rivad refers to one and the
same person, herein affiant.
4. That I am executing this affidavit to attest to the truth of the foregoing
and to use the same for whatever legal purpose it may serve;

2. But ever since childhood, my friends and acquaintances have been
