1. Duly notarized application form, MGB form No. 1-13;

2. Duly notarized supply contract agreement;
3. DTI Accreditation;
4. Project description;
5. Certified true copy of the Governor’s Permit of the supplier;
and
6. Certification issued by PEMO to Leonita C. Gravino as permit
holder
