1. General
The purpose of the document is to adopt uniform practice for receipt,

handling, protection, storage, retention and disposal of samples/items

Scope This is applicable to all test items received by the laboratory.

2. Procedure

2.1 A sample is receive with a proper test request from the customer.

2.2 Upon receipt of the test item, the sample receipt personnel checks the description of sample
from test request or the label and records any abnormalities. If there exists any doubt in
suitability of the item for test or the item does not conform to the description provided, the
customer is consulted for further instructions before start of testing. The details of discussions
are recorded (UTS-FO-031 Evaluation of test request).

2.3 He also verifies the size and suitability of the sample for testing. The sample/item is also
thoroughly examined for any sign of damage/deterioration due to mishandling.

2.4 A unique sample number is assigned to the sample by writing the same on the label of the
sample and also in the sample record kept in the laboratory. This unique identification number is
retained till the sample is disposed. The sample number is also written on customer's test
request, if received. The identification system is to prevent test item, including sub-

Division of it, of being confused physically, when referred to in record or other documents or
while transfer of items within and from the laboratory.

2.5 Details of sample, date of receipt, laboratory code No., and other details are recorded sample
receipt record (UTS-FO-032 Sample Receiving Form).

2.6 Sample receipt personnel reviews the request regarding suitability of carrying out testing of the
test item. In case of any confusion Q.M/T.M. is consulted. After ensuring suitability, the test items
are handed over to the testing persons.

2.7 Samples are stored under proper environmental conditions in such a way that samples do not
get deteriorated. When a sample received is to be stored under specified environmental
condition, these conditions are maintained, monitored and recorded. When a test item or a part
of it has to be held secure, the laboratory has arrangement for storage and security that protects
the integrity of the sample.

2.8 Retention of remnant samples: Remnant of the tested samples is retained in the laboratory for
one month for both internal & external customers. Disposal of remnant samples: Remnant
samples are disposed off at the end of the retention period and record is maintained.

2.9 UTS arranges for sending back the samples forwarded by the customer after completion of the

concerned service, if the same is requested by the customer. Adequate packing and transporting
of the samples is ensured in such cases to ensure that the sample is not damaged.

3. Authorization
1.1. This procedure has been reviewed and determined to conform with the requirements of ISO
17025, this laboratory’s accrediting body, and the requirements of the ISO 17025 Management
System. This procedure is authorized for use.
