APPENDIX C JADUAL KADAR HARGA - KERJA-KERJA SIVIL ‘GENERAL ITEM' - KERJA-KERJA SIVIL METHOD OF MEASUREMENT AND PRICING FACTORS SECTION 1 -GENERAL ITEMS La LL CONTRACTUAL REQUIREMENTS Performance Bond Unit The unit of measurement shall be Lump Sum. Measurement ‘The measurement for payment shall be made as follows: (a) In the first interim payment, the Contractor shall be paid either the actual premium paid by him upon submission of the receipts or the tendered lump sum for the item whichever is the lesser. The receipts of premium paid must be from the company (Bank/Insurance/Finance Company) issuing the guarantees; (&) The remaining sum (if any) shall be paid in each subsequent interim payment in proportion to the total period tapsed to the Contract Period (excluding Defects Liability Period). Insurances Unit The unit of measurement for the following Insurances shall be Lump Sum: (@) Insurance of Works including Insurance Against Personal Injuries and Damage to Properties (®) socso Measurement ‘The measurement for payment shall be made as follows: (2) In the first interim payment, the Contractor shall be paid either the actual premium paid by him upon submission of the receipts or the tendered lump sum for the item whichever is the lesser. Measurement Lump sum payment of 0.125% of the Contract Sum in compliance with the Construction Industry Development Board requirement shall be made upon production of official receipt issued by the Construction Industry Development Board to indicate evidence of payment. SO’S REQUIREMENTS Accommodation for the $Q’s Staff Unit The unit of measurement for the establishment, maintenance and removal of the following accommodation shall be Lump Sum. 12.1.1 Office 1.2.1.2 Laboratories ‘Measurement ‘The measurement for payment shall be made as follows: (@) 60% on completion of the facilities as specified; (6) 20% for maintenance which shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period); (© 20% on removal. ing Factors ‘The item for establishment and removal of accommodation shall in accordance with the Preambles to Bill of Quantities General Direction include: (@) provision and preparation of site; (b) foundations and bases; (©) fences, notice and direction boards; (@ vehicle access, hard standing, parking area and foot paths; MM1/2 (©) office furnishing, fittings, supplies and initial consumable stores as specified; (®) provision of telephones and extension as specified (where applicable); (g) water, sanitation, power, air conditioning and lighting services (h) compliance with local building by-laws; (@ depreciation of buildings, furnishings and fittings; @__ receiving back from the SO and removing buildings, furniture, fittings and supplies off site; (k) disconnecting and removing services and sealing off disused services; () demolishing and removing off site temporary accommodation, vehicle access, hard standing, parking areas, footpath, fences, notices and direction ‘boards; (m) disposal of surplus materials; (n) reinstatement of the site; (0) removal and disconnecting of telephone (where applicable). The item for maintenance of accommodation shall in accordance with the Preambles to Bill of Quantities General Direction include: (a) rental charges including telephone rental (but excluding charges on telephone calls); (b) fees and charges levied by the respective authorities and maintenance of power, lighting, water, sanitation, air conditioning and sewerage disposal; (©) maintenance of buildings, services, fences, notice and direction boards, vehicle access, parking areas, hard standing end footpaths; (@ maintenance of furnishings and fittings and supplies; (©) maintenance of telephone (where applicable); (f) cleaning accommodation including pest control; (g) moving and re-establishing portable office accommodation as required up to a maximum of 3 moves; (h) replenishment of consumable stores; (i) maintenance of all access for use under all weather conditions. MMI/3 1.22 1.2.2.1 1.2.22 Services for the SO’s Staff Transport Services Unit The unit of measurement for the provision of suitable transport service shall be Lump Sum. Measurement The measurement for payment shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period) commencing upon when the transport service is made available. Pri Facto ‘The item shall in accordance with the Preambles to Bill of Quantities General Direction include: (@) provision of the vehicle type as specified together with comprehensive insurance and road tax for every vehicle; (b) vehicles are accident free; (©) certification from Puspakom at every six months; (@)_ transport is readily available for the use of S.0.’s staff, (©) depreciation Operation and Maintenance of Vehicles Unit The unit of measurement for operation and maintenance of vehicles shall be Lump Sum. Measurement The measurement for payment shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period) commencing upon when the transport service is made available. Pricing Factors ‘The item shall in accordance with the Preambles to Bill of Quantities General Direction include: (a) taxing for use on public highway and for carriage of goods and samples; MMU4 1.23 1.23.1 (b) comprehensive insurance to cover all drivers and passengers and the carriage of goods and sample throughout the Contract Period (excluding Defects Liability Period); (©) provision of suitable replacement including equipment when regular vehicle is not available or unserviceable for more thar. 24 hours; (@ maintenance in a roadworthy condition and in conformity with the vehicle manufacturer’s recommendations; (©) fuel, oil and lubricant; (Q) validity of road tax throughout Contract Period (excluding Defects Liability Period), (g) keeping the vehicles clean inside and out. Equipments for use by the SO’s Staff Laboratory Equipments Unit The unit of measurement for the provision and maintenance of laboratory equipments shall be Lump Sum: ‘Measurement ‘The measurement for payment shall be made as follows: (2) 60% on receipt and approval of all the equipments supplied and installed; (>) 40% for maintenance of the equipments which shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period). Pricing Factors The item for provision of laboratory equipment shall in accordance with the Preambles to Bill of Quantities General Direction includ (@) provision of all equipments as listed in the Specification; (b) depreciation; (©) _iwstallation and calibration of the equipments; (@)_ receiving back from SO and removing equipments off site; MMI/S 12.3.2 The item for maintenance of laboratory equipment shall in accordance with the Preambles to Bill of Quantities General Direction include: (@) maintenance of the equipments to a working order condition; (b) provision of suitable replacement when the regular equipment is not available for more than 24 hours; (©) replenishment of consumable items; Office Equipments Unit The unit of measurement for the provision and maintenance of the following office equipments (where applicable) shall be Lump Sum. (@ Computer (b) Photocopy machine (©) Facsimile machine (4) Printer (e) Digital camera © Scanner (g) Overhead projector ‘Measurement The measurement for payment shall be made as follows: (@) 60% upon the complete delivery and commissioning of the equipments as specified: (>) 40% for maintenance of the equipments which shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period). Pricing Factors The item for provision of office equipments shall in accordance with the Preambles to Bill of Quantities General Direction include: (a) provision, delivery to S.0.’s office and commissioning of equipments and accessories/peripherals as listed in the Specification; (b) in the case of computers, the original and legal copies of software as specified: (©) iim the case of facsimile machine, the provision of telephone line and connection; MMI/6 1.24 @ depreciation; (©) removal from the S.0.’s office and revert back to the Contractor upon issuance of the certificate of practical Completion. The item for maintenance of office equipments shall in accordance with the Preambles to Bill of Quantities General Direction include: (2) maintenance of all equipment and accessories; (b) rental charges (where applicable); (©) payment for telephone charges for faxes is not included; (@ repair cost and cost of replacement in case of breakdown; (©) regular servicing of all equipment, with suitable replacement when the regular equipment is not available for more than 24 hours; (© provision and replenishment of consumable items; Attendance for the SO’s Staff Unit The unit of measurement for the following attendance for the S.O.’s staff shalll be Lump Sum. 124.1 Drivers; 12.4.2 Laboratory Assistants Measurement ‘The measurement for payment shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period), ing Factors The item shall in accordance with the Preambles to Bill of Quantities General Direction include (@) the wages and other emoluments paid to the operative including payment for overtime; (b) working outside the Contractor's normal working hours if so required by the SO; MM1/7 12.5 (©) cost and expenses incurred consequent upon the employment or hiring of the operative including payment of Employee Provident Fund, insurance, medical and hospitalization; (a) _ replacement of unsuitable personnel. Measurement ‘The measurement for payment shall be made as follows: (a) 80% on completion of installation; (0) 20% on removal. Pricing Factors ‘The item shall be in accordance with the Preambles to Bill of Quantities General Direction include: (@) excavation in any material including rock and reinforced conerete and loading into transport, upholding the sides and keep the earthworks free of water; (b) backfilling and compaction; (©) support; (d) formwork; (@) _ in-situ concrete; (© disposal of surplus material; (g) painting, high intensity reflectorise aluminium sheeting and illumination; (h) cleaning, maintaining and repairing; MM1/9 1.2.6.2 1.2.6.3 (i) _ erection, dismantling and removing from sit (reinstatement of surfaces; (Kk) payment of all charges including obtaining permission, licenses and fees. etc; Progress Photograph vit The unit of measurement for progress photographic record of work shall be Lump Sum determined by the number of set. Measurement Each set shall comprise 2 prints of each exposure in 3R size, minimum of 36 exposure film taken on any one flight or visit to site. Each set of slides shall comprise 24 slides. Each set of C.D. shall comprise of two C.D.s containing all the selected progress photographs for the month. ‘The measurement for payment for the progress photographs, progress slides and photographs in compact disk (CD) format shall be made of the sets furnished and accepted by the SO paid in each imerim peyment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period). Pricing Factors ‘The item shall be in accordance with the Preambles to Bill of Quantities General Direction include: (@) film, development of film, prints, proofs, slides and documenting in suitable album; (b) delivery and supply to the S.0.’s office; (©) identification marking on the prints and slides; (4) mounting on manila card for display purposes as directed by the S.0. (©) provision of camera and competent photographer; Performance Report Unit The unit of measurement shall be Lump Sum determined by the number of set. Measurement The measurement for payment shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period), Pricing Factors (@) item on implementing the CPM programme for planning of resources and progress of work as specified in the Specification and should include: trained and qualified personnel to maintain and update the programme planning using the CPM programme; providing softwares for JKR, which is to be retained by JKR after the completion of the project. The softwares to be utilized is - the latest version of Microsoft Project 2000 or equivalent for projects costing less than RM30 million; MML/I1 12.6.5 - Primavera Project Planning (P3e) or equivalent for projects costing more than RM50 million or projects which incorporate a lot of scopes and milestones. (b) carry out regular check of progress including updating all information and maintain the planned programme periodically or as and when instructed by the S.0.; (© submission of the reports to JKR on computer medium and printed hard copies periodically or as and when instructed by the 8. Pricing Factors The item shall be in accordance with the Preambles to Bill of Quantities General Direction include: (@) preparation and submission of as-built drawings for approval by the $.0. and final ROW surveyed drawing by an approved licensed Land Surveyor in tracing, print, CAD format in computer CD as detailed in the Specification; (®) providing labour, tools and equipment necessary to take measurement and obtaining such information on site as are required by the S.O. for the preparation of as-built drawings; (©) confirming correctness of as-built drawings; (preparation of monthly periodic updating as work progress for inspection byS.05 (©) preparation and submission of road asset inventory comprising culvert and bridges, slope protection and road furniture in the number of set specified. MMI/12, 12.66 1.2.7 1.28 ‘Clearing Up Of Site Unit The unit of measurement for clearing up of site on completion of the Works shall ‘be Lump Sum, Measurement The measurement for payment shall be made in the last interim payment upon clearing up of Site to the satisfaction of the S.0.. Prici tor The item for clearing up of site on completion shall in accordance with the Preambies to Bill of Quantities General Direction include: (a) reinstatement of damaged area due to any work and temporary work carried by the Contractor to its original condition; (b) disposal of surplus material, waste etc; (©) clearing of the drainage system. License Security Services Unit ‘The unit of measurement for provision of license security services shall be Lump Sum. Measurement ‘The measurement for payment for Contractor's superintendence shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period). Pricing Factors (@) costs and expenses incurred consequent upon the employment or hiring of the license security service; (b) costs of working 24 hours with 12hours shift. (©) replacement of unsuitable personnel; Quality Assurance Plan MMI/13 1.29 Unit The unit of measurement for provision of quality assurance plan for design and construction shall be Lump Sum. Measurement ‘The measurement for payment for Quality Assurance Plan shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period), Pricing Factors (@) costs and expenses incurred consequent upon the employment or hiring of a suitably qualified and experienced person to act solely as Quality Assurance Officer full time on site including payment of Employee Provident Fund, insurance, medical and hospitalization; (b) costs and expenses incurred consequent upon the employment or hiring of Quality Assurance Team full time on site and required resources for effective operation of the Quality Assurance Plan including payment of Employee Provident Fund, insurance, medical and hospitalization; (®) preparation and submission to the SO a properly documented Quality Assurance Plan according to work procedures set in the Public Works Department Quality Management System and updating and revision during the progress of the work; (© _ submission of monthly reports incorporating all test results, test certificates, photographs and lab report, endorsed by the Quality Assurance Officer; @) preparation of quality report in the form of CD and hardcopy. Location, Temporary Protection and Temporary Relocation _of Public ‘Utility Installations and Other Services Unit The unit of measurement shall be Lump Sum, Measurement The measurement for payment for location, temporary protection and temporary relocation of Public Utility installations and other services shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period). MMI/14 13 The item for location and temporary protection of Public Utility installations and, other services shall in accordance with the Preambles to Bill of Quantities General Direction include: (a) _ locating the positions of the utilities including excavating trial holes of all services; (b) making good any damage caused to the approval of the appropriate Service Authority or owner; (©) arranging with the appropriate Service Authority or owner for all temporary diversions required in connection with the works and keeping the P.D. fully informed ofall such negotiations; (A) providing all labour, materials and plant to expeditiously execute the works; (©) protecting utilities to the requirements of the appropriate Service Authority The item for temporary relocation or diversion of Public Utility installations and other services shall in accordance with the Preambles to Bill of Quantities General Direction include: (@) arranging and liaising with appropriate service Authorities or owner for all temporary diversion; (©) complying with requirements of respective Service Authorities; (©) making good of any damage caused to the approval of the appropriate Service Authority; (@ providing all plants, labour and materials; (©) cutting off and disposal of redundant mains pipes, cables, chambers, covers, etc., (© the credit value of disposed materials as stated in (e) above; {g) provision of a competent and experienced person to liaise and coordinate the services operations with the works; (b) reinstate connection to properties and make good of all services; (@__ allowing for inspection by the respective Service Authorities. CONTRACTOR’S REQUIREMENTS MMI/I5 13.4 Contractor’s Accommodation and Buildings Unit The unit of measurement for establishment, maintenance and removal of the following Contractor’s accommodation and buildings shall be Lump Sum. The item for maintenance of Contractor’s accommodation and buildings inclusive of utilities shall in accordance with the Preambles to Bill of Quantities General Direction include: (@) any costs incurred in the use of the land; (©) rental charges (where applicable); (©) fees and charges for water, power, sewage disposal and communication systems; (4) maintenance of all buildings, vehicle access, parking areas, hard standings and footpaths; (©) maintenance of all office equipment, furniture, fittings and communication systems, workshop tools and equipment; (f) cleaning; {g) replenishment of consumables stores. ‘Temporary Works Unit The unit of measurement for the design, construction, maintenance and removal of temporary works as follows shall be Lump Sum. Contractor’s Supervision Unit The unit of measurement for Contractor’s supervision shall be Lump Sum. Measurement The measurement for payment shall be made in each interim payment in proportion of the total period lapsed to the Contract Period (excluding Defects Liability Period). Pricing Factors ‘The item shall be in accordance with the Preambles to Bill of Quantities General Direction include: (@) the wages and other emoluments paid to the site agent and other personnel necessary for the Works; MMI/18 13.4 (b) working outside the Contractor’s normal working hours if so required by the $.0.; (©) costs and expenses incurred consequent upon the employment or hiring of the site agent and all other personnel including payment of Employee Provident Fund, insurance, medical and hospitalization; (@) replacement of unsuitable site agent and other personnel; (©) substitution of superintendent when the appointed site agent is absent or on leave for more than three (3) consecutive days. Mobilisation and Demobilisation of Construction Plant and Equipments Unit The unit for measurement for mobilization and demobilization of construction plant and equipments shall be Lump Sum. Measurement ‘The measurement for payment shall be made as follows: (@) 60% on completion of mobilisation; (b) 40% on completion of demobilisation Pricing Factors The item for mobilisation of construction plant and equipments shall be in accordance with the Preambles to Bill of Quantities General Direction include: (@) transport to site including loading and unloading at ports and site, places of assembly in Melaysia or other sites to the sites where they are to be used on the works; (b) obtaining any import licenses/permits; (© alltaxes, import duties and other fees; @ assembling, erection and installation. ‘The item for demobilisation of construction plant and equipments shall be in accordance with the Preambles to Bill of Quantities General Direction include: (a) dismantling; (b) transport from site including loading and unloading at ports and site; (©) obtaining any export licenses/permits. MM aia 33 PLACEMENT AND COMPACTION OF EARTH EMBANKMENT Unit : ‘The units of measurement for placement and compaction of suitable fill ‘material or rock fill in embankments and surcharge to embankments where necessary or instructed shall be in cubic metre. jeasurement The units of measurement for placement and compaction of suitable fill materials shall be made of the volume of the embankment measured from the original or stripped or excavated ground levels to the finished, levels, grades and dimensions shown on the Drawings or as directed by the S.0. The line of the top of embankment shall be the formation levels or surcharge levels as shown on the Drawings and for the embankment over soft ground the settled volume as derived and computed by the method as stated below shall be added to the volume above for payment. (@) Prior to the commencement of filling works, rod settlement markers shall be installed by the Contractor at the edge, at the toes and at the centre line of the embankment at the approximately 75 metre intervals or at closer spacings as approved by the 8.0. (b) The measured settlement which is the difference in level between the initial level (to be taken immediately after the compaction of the first layer of fil over the soft ground area) and the final level (taken immediately after the completion to shape and levels of the surcharge work) of the respective rod settlement gauges shall be the settlement of the embankment at those locations; (©) For the purposed of computing the volume of embankment setiled, the first 75mm of the settlement as obtained above shall be subtracted from the measured settlement; (@) The net settlement as derived above shall be assumed linear and shall be used for calculating the volume of embankment settled below the foundation level. - Item Coverage ‘The rate shall include :~ (a) Selection of suitable material, depositing, spreading, leveling, compacting and finishing of embankment or fill including filling in swamps and old water courses. MM 3i9 SECTION 4 - DRAINAGE WORKS 4d 412 SURFACE DRAINS EXCAVATION FOR DRAINAGE Unit: The unit of measurement for excavation of drains if separately measured shall be cubic metres. Measurement Excavation for drains, water courses culverts and foundations if separately measured shall be measured nett and the dimensions for the purpose of measuring shal! be based on the horizontal area at the bottom of the wall, foundation or other structure to be built multiplied by the mean depth. The ‘mean depth shall be taken from the foundation level to the finished surface or formation level as shown on the Drawing or the existing ground level, whichever is lower. The quantities of excavation for open drains and ditches are to be the nett cubic content, in cubic metres, of the void to be to be formed by the removal of excavated in accordance with the specification and Drawings.
