SUBJECT : Request for Downgrading from Most Wanted Person to Watch List

DATE : September 30, 2021

1. References: RHPU 3 list of Criminal Gangs (CGs).

2. In connection with the above cited reference, upon conducting a diligent

surveillance/tracking for the status of Most Wanted Person of PHPT-Bulacan.

3. On September 24, 2021, Intel personnel of this office conducted coordination to

Brgy. Chairman Enrico C. Cruz of Brgy. San Juan, Hagonoy, Bulacan for the update of Luisito G.
Cervantes. Upon checking and information received that the target personality Luisito G.
Cervantes is no longer resides on the said Barangay.

4. On September 30, 2021, Intel personnel of this office conducted coordination to

Brgy Capt. ______, Brgy. San Jose, Baliuag, Bulacan for the update of Daryl Contento y Jaen.
Upon checking and information received that the target personality Daryl Contento y Jaen is no
longer resides on the said Barangay.

5. In support to our claim submitted hereunder is the certification being issued of

different government offices concerning the said person such as;

a. Certification from Barangay Hall of Brgy San Juan, Hagonoy, Bulacan signed by
Hon. Enrico C. Cruz, Brgy. Chairman dated September 24, 2021; and
b. Certification from Barangay Hall of Brgy San Jose, Baliuag, Bulacan signed by
Hon. _______, Brgy. Chairman dated September 30, 2021

6. Upon gathering of all the information, this office is recommending that subject target
personalities be downgraded from Most Wanted Person (Provincial Level) to Watch List. However
this office will continuously monitored as the whereabouts of subject person.
