Discharge Summary, Philhealth forms CSF, CF2
and CF4 from the ROD’s

Tagging in Bizbox ( Indicate admitting

checked before pt. discharged

Once you have 2 copies of discharge

notice coming from the information
department

Ask the patient/ relative if
