3. The teacher may add comments and must sign this report to acknowledge receipt. At the
request of either the teacher or the principal, the teacher and the principal must meet to
discuss the performance appraisal after the teacher receives a copy of this report
4. A copy of this report signed by both the principal and the teacher must be sent to the
appropriate supervisory officer.
5. In preparing the summative report, the principal must:
consider all 16 competencies in assessing the teacher's performance;
provide comments regarding the competencies identified in discussions with the teacher
as most relevant to the teacher's performance appraisal;*
provide an overall rating of the teacher's performance in accordance with the rating scale
recommend professional growth goals and strategies for the teacher's development.
Focus of the classroom observation was on Mathematics. A three part numeracy lesson based on an
template from MathUp. This included whole group, small group and individual activities.

If the teacher receives a Satisfactory rating, the principal is encouraged to provide further feedback
on strengths and possible areas of growth for the teacher.
Alex is an trusted and respected teacher in our school who demonstrates knowledge of how students
learn and factors that influence their achievement. He serves as an excellent role model for his students.
His calm and patient mannerisms have enabled him to develop a strong rapport with his students. Alex
maintains structure and routine in his class using a positive approach with students and engaging them
in active and fun learning strategies. When reminders for appropriate responses are necessary, Alex
maintains his student's dignity while redirecting them in a firm but fair manner. Alex is a collaborative
member of the St. Stephen staff that is eager to share his knowledge and expertise with others.
Professional Growth Goals and Strategies for the Teacher (required, if rating is Satisfactory)
The following professional growth goals and strategies are recommended for the teacher to take
into account in developing his or her Annual Learning Plan (ALP).
Alex has produced an ambitious Annual Learning Plan. The focus of the plan is on leadership in
preparation for potentially entering the Vice Principal stream. He is eager to practice his leadership
skills and has volunteered to act as Teacher Designate should administration be away. He has assumed
a leadership role by acting as co-chair our school’s Social/Staff Wellness Committee. Additionally, he is
co-chair our school’s Safe Schools & Equity Committee and is working collaboratively with staff to
develop equity activities to enhance the school experience for all students.
