I, Mr. SVDNS RAVI TEJA BALLA, S/O VENKATESWARA RAO BALLA,

aged about 32 Years, R/O F-403, SAI RAGHAVA ENCLAVE, SY NO 208/B, BEHIND
SREE DEVI THEATRE, CHANDANAGAR, HYDERABAD-50, Telangana Do hereby
solemnly affirm and state as follows:

1. That my recorded name in School, College and GOVT Identity Certificates is SVDNS RAVI
TEJA BALLA, but I myself popularly known as RAVITEJA BALLA.

2. That by virtue of this affidavit, I changed my name from SVDNS RAVI TEJA BALLA to
RAVITEJA BALLA, and henceforth I shall be known as RAVITEJA BALLA for all purposes.

3. That for the purpose of evidencing such my determination declare that I shall at all times
hereafter in all records, deeds and writings and in all proceedings, dealings and transactions,
private as well as upon all occasions whatsoever, use and sign the name of RAVITEJA BALLA
in place and in substitution of my former name SVDNS RAVI TEJA BALLA.
