SECTION 8 : Performance Report – Occupational Accident / Incident Statistic Records

Following are several items / areas to be consider by the management for improvement: -

A. Safety program will be compulsory for all parties.

B. Management to supply/provide suitable PPE for every workers and recorded.

C. Supervisor/Management to ensure all workers are wearing suitable PPE when doing their tasks.

D. To ensure every workers that work in height (above 3 meter) using Scissor Lift should be operated by well-trained
worker and provided with suitable PPE (Safety Harness). Supplier to conduct training on safe use of Scissor Lift.

E. The Danger Zone around the structure should be adequately fenced off and warning signed posted.

F. Warning signed with already worn-out need to be replace.

G. Pavement/OKU ramps that damaged need to be repaired immediately and to provided hand railing.

H. Tempered glass that crack need to be changed immediately.

To ensure every EMERGENCY DOOR not locked and obstructed.

J. Road devider that is tarnished need to re-painted.

K. All Supervisor and Management must show good example to workers.

L. All Supervisor and Management must ensure that there is no open burning at work area all the
times.
