JOINT AFFIDAVIT OF LEGITIMATION

We, _________________, both Filipinos, of legal ages, married to

each other and residents of ________________, under oath, depose
and state that:

1. We are the biological parents of _______________ born on

__________ at Cebu City as evidenced by the attached copy of
the child’s Birth Certificate, marked as Annex “A”;

2. At the time of his birth, we, the biological parents were not yet
married;

3. At the time of the conception of our child, there was no legal

impediment for us to contract marriage;

4. By virtue of our subsequent marriage, the said child is now

legitimated by operation of law; and

5. We got married on _______________ at ______________ as

evidenced by the attached Marriage Certificate, marked as
Annex “B”;

6. We are executing this Affidavit to attest to the foregoing facts,

for purposes of complying with the requirements in applying
for the legitimation by subsequent marriage of our son,
