I, Aurelio Sisalvo, legal age,and a resident of Ambalatungan, Santiago City though hereby certify and
acknowledges the cash balance amounting to Eighty three thousand pesos (P83, 000) to Arch. Arvin
Siniguian (contractor) for the residential project located Ambalatungan,Santiago City. The said amount
will be given upon the end of the six-month period warranty starting from the turn over date _______ to
________.

Note: The warranty refers only to those repairs which are defective due to material flaws and not
owed to negligence, mishandling, or force majeure.
