1. Reference: PNP MC No. 2017-031 “Revised Guidelines and Procedures
on the Use of Short Messaging System (SMS) in the Reporting of Significant
Incidents dated April 8, 2017”.

This pertains to the observation that PNP Office/Unit Commanders report
via SMS incidents not following the prescribed format and sometimes without regard
to syntax and grammar.

In this regard, the basic guidelines to be followed in the reporting of
shall be observed by all PNP Office/Unit Commanders. Reporting of
incident must be prompt, factual and validated by listing down the necessary
information in itemized or outline form. This shall answer the necessary requirements
of the five
eres
What - Indicate the type of incident. This includes incidents, disasters,
emergencies and socio political activities happening that needs to be
passed on to higher offices;
b. When - Be precise, as much as possible, indicating the time and date
of the incident. Use AM or PM instead of military time;
c. Where - State where the incident occurred. e.g. Along Juan Luna St,
Brgy. Bagong Lipunan, Quezon City;
d. Who - Write down the names of all those who were involved in the
incident, victims, suspects, witnesses (if any);
e. Why — Although optional, answers a question, among others, as to the
motive of the incident; and
f. How - The outline on how the incident happened shall be indicated to
‘show the description of the chronology of events that led to the incident
and the circumstances thereafter. This shall include the actions taken
by responding police officers and the initial result of the investigation
conducted.4. Further, indicate the complete name and the contact number of the
Investigator-on-Case (IOC). Likewise, use only the common and accepted
abbreviation.

‘Where: 175 Laong Laan St., Brgy. Bagong Lipunan, Quezon City
‘Who: Victim — John Michael B. Cruz, 23 yrs old, Single, resident of Bray. Palanan, Makati City.
‘Suspect ~ Richard T, Santos, 30 yrs old, Married, resident of Brgy. Bagong Lipunan,
‘Quezon City.
Why: Based on initial investigation the victim has an Illicit affair with the wife of the suspect.
How: The victim was invited by his friend for a drinking session at the resident of SK Kagawad
Michael L. Gregorio of the said Brgy. not knowing that the suspect was also present.
When suddenly a heated argument ensued between the victim and the suspect wherein
the suspect suddenly shot the victim heating him in different parts of his body. The
suspect immediately fled after the incident. Responding police officers of Kamuning
Police Station, QCPD immediately brought the victim to East Avenue Medical Center but
was pronounced Dead on Arrival (DOA) by Dr. Rodolfo B. Dimaculangan the attending
physician, Ongoing investigation by the Kamuning Police Station. IOC PSSgt Ferdinand

Anti-lllegal Drug Operation (Buy Bust)

Suspect - RolandAbellaGlorina, 40 yrs old, married and a resident of
Brgy. Kainaman, Cebu City.
The arrested suspect is a member of a big time illegal drug group
operating in Cebu City.
A joint buy bust operation was conducted by personnel of Police Station 7
of CCPO, Special Operations Unit 5, PNP DEG and RID 7 against the
suspect resulting to his arrest and the confiscation of one (1) large knot-
tied transparent plastic bag and one (1) small heat sealed transparent
plastic sachet (buy bust item) all containing white crystalline substance
believed to be shabu with estimated weight of 103 grams and DDB value
of Php 700,400.00. A Php 2,000.00 (buy bust money) was also recovered
from his possession. The suspect is currently detained at Police Station 7
lock-up jail while documents for the filing of complaint are being prepared.

Implementation of Search Warrant

Suspect - German Contimplado Jose, 24 yrs old, single and a resident of
Brgy. San Cristobal, Quezon City.
The operation stemmed from a complaint filed by the Philippine Airlines
(PAL) through its representative, Charlie De Leon Juan, PAL Customer
Management Officer, regarding a bomb threat received through e-mail
from a certain “SAJUD DINSALA". Accordingly, this office was able to
identify and locate the sender through the IP address used in sending the
malicious e-mail thus the conduct of operation.
A joint operation was implemented by QCPD PS3 and PNP Anti-
Cybercrime Group personnel by virtue of Search Warrant No. 12345
issued and signed by Hon. JUANCHO DELA CERNA LIM, Presiding
Judge, RTC Br 20, for violation of PD 1727 (Malicious Dissemination of
False Information of the Willful Making of any Threat Concerning Bombs,
Explosives or any Similar Devices or Means of Destruction and Imposing
Penalties Thereof) in relation to Republic Act No. 10175 otherwise known
as the “Cybercrime Prevention Act of 2012 that resulted to the arrest of
the suspect and the seizure of one (1) Samsung Duos Galaxy 10 cellular
phone and one (1) PLOT Home Fibr Modem/Router. The suspect,
together with the seized items, was brought to the office of PNP ACG
while documents for the filing of complaint are being prepared. IOC -
