A RARE CASE REPORT OF : PULMONARY AGENESIS IN SIXTEEN-DAY OLD

Background
Pulmonary agenesis is an extremely rare congenital lung abnormality that can associated with
congenital malformation of the cardiovascular, skeletal, gastrointestinal or genitourinary
system. When there is complete absence of a main stem bronchus, the anomaly must occur
close to the end of the first lunar month. This lung abnormality may occur in various ways :
gene mutations, environmental interference, infection, irradiation, mechanical factors in utero

Objective
To investigate the chest x-ray examination, lung ultrasonography and chest computed
tomography examination of pulmonary agenesis.

Method
We report a sixteen-day-old boy with respiratory distress and cyanosis was referred to our
department for chest x-ray examination, lung ultrasonography and chest computed
tomography examination. Prenatal history was born from G2P1A0 mother, 32 weeks of
pregnancy, sectio caesaria because of premature rupture of membrane. Physical examination
showed cyanosis, chest asymetry and dullness on the right hemithorax. The case report is
based on chest x-ray examination, lung ultrasonography and chest computed tomography
examination.

Result
The chest x-ray showed a opacity on the right lung and shifting of the mediastinum to the
right. Lung ultrasonography showed minimal structure of the right lung. And the chest
computed tomography showed complete absence of bronchus, lung tissue and right
pulmonary artery. This patient was treated conservatively with medication and discharged
after 8 days.

Conclusion
Pulmonary agenesis is an extremely rare congenital lung abnormality. However the diagnosis
made by chest computed tomography examination could show the pulmonary agenesis more
clearly than lung ultrasonography and chest x-ray.
