As per Reference cited above Smt D. Sri Lakshmi C.O ( Community Organiser) must
gather the information from ANMs and Ashas , Verify and send them to Higher Authorities in
Stipulated time. And also Co ordinate with RRT team/other Supervisors deputed to UFWC to
confirm the Details of Patients who comes to Repalle to prevent the Local Spread of the COVID-19.

Hence the above said Individual is Strictly Instructed to Check the Emails and
Intimate to Medical Officer and send them in Stipulated Time. She is held responsible for in case of
delay and will be intimated to higher authorities for further action under CCA rules.
