JOINT AFFIDAVIT

WE, MELVIN P. JAVIER and ROSALYN P. ESGUERRA, both of legal ages,

Filipinos and with residence and postal address at Barangay Lipatan, Balayan,
Batangas, respectively, after having been sworn to in accordance with the law
hereby depose and state that:

1. We personally know JOSE SALAZAR CASTROMERO who was born on April 11,
1951 evidenced by his Birth Certificate issued by the Office of the Civil
Registrar, Balayan, Batangas;

2. It is also the date of birth indicated in his Certificate of Death;

3. We came to know that in his E1 SSS Records, his date of birth was misstated
as April 11, 1953;

4. We declare, confirm and affirm that he was born on the 11 th day of the April
1951 because we are quite close to each other;

5. He has been using the said birth date in some of his identification and
documents and transactions;

6. We declare that the date April 11, 1951 and April 11,1953 refer to the date
of birth of one person, only to JOSE SALAZAR CASTROMERO;

7. We are not related by affinity or consanguinity to the above mentioned

person; and

8. We are executing this affidavit to attest to the truthfulness of the foregoing

statement and for all legal intents and purposes it may serve.

9. As such, we execute this Affidavit to attest to the truth of the above

statement and for whatever lawful purpose it may serve.
