GOPRO ON-BOARDING COMMITMENT FORM

For the Candidate AUM:

I commit to participate in the Provincial “GoPro” CAUM program that will enable me to build my own
team of Productive and Protection-focused Financial Advisors. The success of the program will hinge on
a strong collaboration between Agency Leaders and the Company. Hence moving forward, I commit to
the following:
A. Focus on the recruitment and activation;
B. Participate actively in the monthly training to support my development;
C. Attend the WEEKLY business review with my Agency Leader and ASG partner.

To support my business plans, I am fully aware that I will be given three (3) months to complete the
following requirements for me to be appointed as AUM under the GoPro Program:

Once I meet the above requirements within the campaign period, I will be enrolled in the GoPro AUM
program. Aside from the standard AUM benefits, I receive an additional monthly start up fund as long as
