Secure transfer order from the

Resident on Duty

Secure 2 copies of transfer form

(patient’s chart and information)

Inform the patient and (if with relative)

regarding the transfer to another ward

Arrange/ Check the chart in order; update the

standing order sheet and accentuate the
pending labs that needs to be facilitate

Inform the name,

age , AP,
classification,
Staff notifies the receiving nurse’s station of diagnosis ,
transfer contraptions and to
where will be the
patient will be
transferred ( e.g ;
isolation or ICU)

In- charge nurse accompanies and endorses

patient to the floor nurse

Inform the patient’s details which also

includes classification of patient,
attending physician diagnosis of
patient, diet, pending laboratories
and special procedures

Endorse to the receiving nurse the

*NEGATIVE OFFICIAL RT-PCR of
the patient and relative

Inform the ROD of the patient’s

transfer to other ward
